Registration Form

First Semester: Sept. 13, 14, 15 to Dec. 6, 7, 8, 2011
Second Semester: Jan. 10, 11, 12 to April 24, 25, 26, 2012

Registration fee of $115 for the year is required with registration. A refund of $55.00 is available if your child is not able to attend second semester.

Wednesday evening (6:15-7:45 PM): Registration fee of $90 for the year is required with registration for Wednesday evening. Family supper is at 5:30 PM and programs are offered for children and adults.

Please send your registration fee to Elmhurst Christian Reformed Church at 155 W Brush Hill Rd, Elmhurst, IL 60126 within one week after registering online to secure your child's enrollment. You may also call (630) 600-5049 and inquire about our scholarship fund that we have available.

When you register online, you should receive a response from the Director shortly, but if one or two weeks pass, and you have not received a reply, please call (630) 600-5049 to assure that your e-mail had gone through. Thank you for your cooperation.
